c869259881 ixgbevf: Mailbox improvements
Improve reliability of the mailbox communication and remove
its potential flaws that may lead to the undefined or faulty behavior.

Recently some users reported issues on ESX with 10G Intel NICs which were
found to be caused by incorrect implementation of the PF-VF mailbox
communication.

Technical investigation highlighted areas to improve in the communication
between PF or VF that wants to send the message (sender) and the other
part which receives the message (receiver):

 - Locking the mailbox when the sender wants to send a message
 - Releasing the mailbox when the communication ends
 - Returning the result of the mailbox message execution

51dce24bcd net: intel: Cleanup the copyright/license headers
After many years of having a ~30 line copyright and license header to our
source files, we are finally able to reduce that to one line with the
advent of the SPDX identifier.

Also caught a few files missing the SPDX license identifier, so fixed
them up.

8d055cc0c8 ixgbevf: make use of BIT() macro to avoid shift of signed values
Also cleanup a case where we're bit shifting a value into place, and use
an unsigned constant. Make use of the unsigned postfix in places where
BIT() macro is not appropriate.

29d37fa162 ixgbevf: merge ixgbevf_tx_map and ixgbevf_tx_queue into a single function
This change merges the ixgbevf_tx_map call and the ixgbevf_tx_queue call
into a single function.  In order to make room for this setting of cmd_type
and olinfo flags is done in separate functions.

de02decb33 ixgbevf: create function for all of ring init
This patch creates new functions for ring initialization,
ixgbevf_configure_tx_ring() and ixgbevf_configure_rx_ring(). The work done
in these function previously was spread between several other functions and
this change should hopefully lead to greater readability and make the code
more like ixgbe.  This patch also moves the placement of some older functions
to avoid having to write prototypes.  It also promotes a couple of debug
messages to errors.

56e94095ef ixgbevf: Add VF DCB + SR-IOV support
This change adds support for DCB and SR-IOV from the VF.  With this change
in place the VF will correctly use a traffic class other than 0 in the case
that the PF is configured with the default user priority belonging to a
traffic class other than 0.

3118678518 ixgbevf: Add support for VF API negotiation
This change makes it so that the VF can support the PF/VF API negotiation
protocol.  Specifically in this case we are adding support for API 1.0
which will mean that the VF is capable of cleaning up buffers that span
multiple descriptors without triggering an error.

70a10e258c ixgbevf: Consolidate Tx context descriptor creation code
There is a good bit of redundancy between the Tx checksum and segmentation
offloads.  In order to reduce some of this I am moving the code for
creating a context descriptor into a separate function.

5f3600ebe2 ixgbevf: Use igb style interrupt masks instead of ixgbe style
The interrupt registers accessed in ixgbevf are more similar to the igb
style registers than they are to the ixgbe style registers.  As such we
would be better off setting up the code for the EICS, EIMS, EICS, EIAM, and
EIAC like we do in igb instead of ixgbe.
