Virtual Trust Levels (VTL) helps enable Hyper-V Virtual Secure Mode (VSM)
feature. VSM is a set of hypervisor capabilities and enlightenments
offered to host and guest partitions which enable the creation and
management of new security boundaries within operating system software.
VSM achieves and maintains isolation through VTLs.
Add early initialization for Virtual Trust Levels (VTL). This includes
initializing the x86 platform for VTL and enabling boot support for
secondary CPUs to start in targeted VTL context. For now, only enable
the code for targeted VTL level as 2.
When starting an AP at a VTL other than VTL0, the AP must start directly
in 64-bit mode, bypassing the usual 16-bit -> 32-bit -> 64-bit mode
transition sequence that occurs after waking up an AP with SIPI whose
vector points to the 16-bit AP startup trampoline code.

With changes to how Hyper-V guest VMs flip memory between private
(encrypted) and shared (decrypted), creating a second kernel virtual
mapping for shared memory is no longer necessary. Everything needed
for the transition to shared is handled by set_memory_decrypted().
As such, remove swiotlb_unencrypted_base and the associated
code.

Hyper-V guests on AMD SEV-SNP hardware have the option of using the
"virtual Top Of Memory" (vTOM) feature specified by the SEV-SNP
architecture. With vTOM, shared vs. private memory accesses are
controlled by splitting the guest physical address space into two
halves.
vTOM is the dividing line where the uppermost bit of the physical
address space is set; e.g., with 47 bits of guest physical address
space, vTOM is 0x400000000000 (bit 46 is set). Guest physical memory is
accessible at two parallel physical addresses -- one below vTOM and one
above vTOM. Accesses below vTOM are private (encrypted) while accesses
above vTOM are shared (decrypted). In this sense, vTOM is like the
GPA.SHARED bit in Intel TDX.
Support for Hyper-V guests using vTOM was added to the Linux kernel in
two patch sets[1][2]. This support treats the vTOM bit as part of
the physical address. For accessing shared (decrypted) memory, these
patch sets create a second kernel virtual mapping that maps to physical
addresses above vTOM.
A better approach is to treat the vTOM bit as a protection flag, not
as part of the physical address. This new approach is like the approach
for the GPA.SHARED bit in Intel TDX. Rather than creating a second kernel
virtual mapping, the existing mapping is updated using recently added
coco mechanisms.
When memory is changed between private and shared using
set_memory_decrypted() and set_memory_encrypted(), the PTEs for the
existing kernel mapping are changed to add or remove the vTOM bit in the
guest physical address, just as with TDX. The hypercalls to change the
memory status on the host side are made using the existing callback
mechanism. Everything just works, with a minor tweak to map the IO-APIC
to use private accesses.
To accomplish the switch in approach, the following must be done:
* Update Hyper-V initialization to set the cc_mask based on vTOM
and do other coco initialization.
* Update physical_mask so the vTOM bit is no longer treated as part
of the physical address
* Remove CC_VENDOR_HYPERV and merge the associated vTOM functionality
under CC_VENDOR_AMD. Update cc_mkenc() and cc_mkdec() to set/clear
the vTOM bit as a protection flag.
* Code already exists to make hypercalls to inform Hyper-V about pages
changing between shared and private. Update this code to run as a
callback from __set_memory_enc_pgtable().
* Remove the Hyper-V special case from __set_memory_enc_dec()
* Remove the Hyper-V specific call to swiotlb_update_mem_attributes()
since mem_encrypt_init() will now do it.
* Add a Hyper-V specific implementation of the is_private_mmio()
callback that returns true for the IO-APIC and vTPM MMIO addresses

Hyper-V should never specify a VM that is a Confidential VM and also
running in the root partition. Nonetheless, explicitly block such a
combination to guard against a compromised Hyper-V maliciously trying to
exploit root partition functionality in a Confidential VM to expose
Confidential VM secrets. No known bug is being fixed, but the attack
surface for Confidential VMs on Hyper-V is reduced.

Child partitions are free to allocate SynIC message and event page but in
case of root partition it must use the pages allocated by Microsoft
Hypervisor (MSHV). Base address for these pages can be found using
synthetic MSRs exposed by MSHV. There is a slight difference in those MSRs
for nested vs non-nested root partition.

If x2apic is not available, hyperv-iommu skips remapping
irqs. This breaks root partition which always needs irqs
remapped.
Fix this by allowing irq remapping regardless of x2apic,
and change hyperv_enable_irq_remapping() to return
IRQ_REMAP_XAPIC_MODE in case x2apic is missing.
Tested with root and non-root hyperv partitions.

In newer versions of Hyper-V, the x86/x64 PMU can be virtualized
into guest VMs by explicitly enabling it. Linux kernels are typically
built to automatically enable the hardlockup detector if the PMU is
found. To prevent the possibility of false positives due to the
vagaries of VM scheduling, disable the PMU-based hardlockup detector
by default in a VM on Hyper-V. The hardlockup detector can still be
enabled by overriding the default with the nmi_watchdog=1 option on
the kernel boot line or via sysctl at runtime.
This change mimics the approach taken with KVM guests in
commit 692297d8f9 ("watchdog: introduce the hardlockup_detector_disable()
function").
Linux on ARM64 does not provide a PMU-based hardlockup detector, so
there's no corresponding disable in the Hyper-V init code on ARM64.

Hyper-V provides host version number information that is output in
text form by a Linux guest when it boots. For whatever reason, the
formatting has historically been non-standard. Change it to output
in normal Windows version format for better readability.
Similar code for ARM64 guests already outputs in normal Windows
version format.

The kernel derives the confidential computing platform
type it is running as from sme_me_mask on AMD or by using
hv_is_isolation_supported() on HyperV isolation VMs. This detection
process will be more complicated as more platforms get added.
Declare a confidential computing vendor variable explicitly and set it
via cc_set_vendor() on the respective platform.

hyperv Isolation VM requires bounce buffer support to copy
data from/to encrypted memory and so enable swiotlb force
mode to use swiotlb bounce buffer for DMA transaction.
In Isolation VM with AMD SEV, the bounce buffer needs to be
accessed via extra address space which is above shared_gpa_boundary
(E.G 39 bit address line) reported by Hyper-V CPUID ISOLATION_CONFIG.
The access physical address will be original physical address +
shared_gpa_boundary. The shared_gpa_boundary in the AMD SEV SNP
spec is called virtual top of memory(vTOM). Memory addresses below
vTOM are automatically treated as private while memory above
vTOM is treated as shared.
Swiotlb bounce buffer code calls set_memory_decrypted()
to mark bounce buffer visible to host and map it in extra
address space via memremap. Populate the shared_gpa_boundary
(vTOM) via swiotlb_unencrypted_base variable.
The map function memremap() can't work in the early place
(e.g ms_hyperv_init_platform()) and so call swiotlb_update_mem_
attributes() in the hyperv_init().

Explicitly check for MSR_HYPERCALL and MSR_VP_INDEX support when probing
for running as a Hyper-V guest instead of waiting until hyperv_init() to
detect the bogus configuration. Add messages to give the admin a heads
up that they are likely running on a broken virtual machine setup.
At best, silently disabling Hyper-V is confusing and difficult to debug,
e.g. the kernel _says_ it's using all these fancy Hyper-V features, but
always falls back to the native versions. At worst, the half baked setup
will crash/hang the kernel.

Commit dce7cd6275 ("x86/hyperv: Allow guests to enable InvariantTSC")
added the support for HV_X64_MSR_TSC_INVARIANT_CONTROL. Setting bit 0
of this synthetic MSR will allow hyper-v guests to report invariant TSC
CPU feature through CPUID. This comment adds this explanation to the code
and mentions where the Intel's generic platform init code reads this
feature bit from CPUID. The comment will help developers understand how
the two parts of the initialization (hyperV specific and non-hyperV
specific generic hw init) are related.

The check for whether hibernation is possible, and the enabling of
Hyper-V panic notification during kexec, are both architecture neutral.
Move the code from under arch/x86 and into drivers/hv/hv_common.c where

Architecture independent Hyper-V code calls various arch-specific handlers
when needed. To aid in supporting multiple architectures, provide weak
defaults that can be overridden by arch-specific implementations where
appropriate. But when arch-specific overrides aren't needed or haven't
been implemented yet for a particular architecture, these stubs reduce
the amount of clutter under arch/.
No functional change.

The code to allocate and initialize the hv_vp_index array is
architecture neutral. Similarly, the code to allocate and
populate the hypercall input and output arg pages is architecture
neutral. Move both sets of code out from arch/x86 and into
utility functions in drivers/hv/hv_common.c that can be shared
by Hyper-V initialization on ARM64.
No functional changes. However, the allocation of the hypercall
input and output arg pages is done differently so that the
size is always the Hyper-V page size, even if not the same as
the guest page size (such as with ARM64's 64K page size).

Marking TSC as unstable has a side effect of marking sched_clock as
unstable when TSC is still being used as the sched_clock. This is not
desirable. Hyper-V ultimately uses a paravirtualized clock source that
provides a stable scheduler clock even on systems without TscInvariant
CPU capability. Hence, mark_tsc_unstable() call should be called _after_
scheduler clock has been changed to the paravirtualized clocksource. This
will prevent any unwanted manipulation of the sched_clock. Only TSC will
be correctly marked as unstable.

kernel.h is being used as a dump for all kinds of stuff for a long time.
Here is the attempt to start cleaning it up by splitting out panic and
oops helpers.
There are several purposes of doing this:
- dropping dependency in bug.h
- dropping a loop by moving out panic_notifier.h
- unload kernel.h from something which has its own domain
At the same time convert users tree-wide to use new headers, although for
the time being include new header back to kernel.h to avoid twisted
indirected includes for existing users.

Previously, to detect nested virtualization enlightenment support,
we were using HV_X64_ENLIGHTENED_VMCS_RECOMMENDED feature bit of
HYPERV_CPUID_ENLIGHTMENT_INFO.EAX CPUID as docuemented in TLFS:
"Bit 14: Recommend a nested hypervisor using the enlightened VMCS
interface. Also indicates that additional nested enlightenments
may be available (see leaf 0x4000000A)".
Enlightened VMCS, however, is an Intel only feature so the above
detection method doesn't work for AMD. So, use the
HYPERV_CPUID_VENDOR_AND_MAX_FUNCTIONS.EAX CPUID information ("The
maximum input value for hypervisor CPUID information.") and this
works for both AMD and Intel.

Microsoft Hypervisor expects the logical processor index to be the same
as CPU's index during logical processor creation. Using cpu_physical_id
confuses hypervisor's scheduler. That causes the root partition not boot
when core scheduler is used.
This patch removes the call to cpu_physical_id and uses the CPU index
directly for bringing up logical processor. This scheme works for both
classic scheduler and core scheduler.

Linux has support for free page reporting now (36e66c554b) for
virtualized environment. On Hyper-V when virtually backed VMs are
configured, Hyper-V will advertise cold memory discard capability,
when supported. This patch adds the support to hook into the free
page reporting infrastructure and leverage the Hyper-V cold memory
discard hint hypercall to report/free these pages back to the host.

STIMER0 interrupts are most naturally modeled as per-cpu IRQs. But
because x86/x64 doesn't have per-cpu IRQs, the core STIMER0 interrupt
handling machinery is done in code under arch/x86 and Linux IRQs are
not used. Adding support for ARM64 means adding equivalent code
using per-cpu IRQs under arch/arm64.
A better model is to treat per-cpu IRQs as the normal path (which it is
for modern architectures), and the x86/x64 path as the exception. Do this
by incorporating standard Linux per-cpu IRQ allocation into the main
SITMER0 driver code, and bypass it in the x86/x64 exception case. For
x86/x64, special case code is retained under arch/x86, but no STIMER0
interrupt handling code is needed under arch/arm64.
No functional change.

For now we can use the privilege flag to check. Stash the value to be
used later.
Put in a bunch of defines for future use when we want to have more
fine-grained detection.

If bit 22 of Group B Features is set, the guest has access to the
Isolation Configuration CPUID leaf. On x86, the first four bits
of EAX in this leaf provide the isolation type of the partition;
we entail three isolation types: 'SNP' (hardware-based isolation),
'VBS' (software-based isolation), and 'NONE' (no isolation).

Currently the kexec kernel can panic or hang due to 2 causes:
1) hv_cpu_die() is not called upon kexec, so the hypervisor corrupts the
old VP Assist Pages when the kexec kernel runs. The same issue is fixed
for hibernation in commit 421f090c81 ("x86/hyperv: Suspend/resume the
VP assist page for hibernation"). Now fix it for kexec.
2) hyperv_cleanup() is called too early. In the kexec path, the other CPUs
are stopped in hv_machine_shutdown() -> native_machine_shutdown(), so
between hv_kexec_handler() and native_machine_shutdown(), the other CPUs
can still try to access the hypercall page and cause panic. The workaround
"hv_hypercall_pg = NULL;" in hyperv_cleanup() is unreliabe. Move
hyperv_cleanup() to a better place.

When a Linux VM runs on Hyper-V, if the VM has CPUs with >255 APIC IDs,
the CPUs can't be the destination of IOAPIC interrupts, because the
IOAPIC RTE's Dest Field has only 8 bits. Currently the hackery driver
drivers/iommu/hyperv-iommu.c is used to ensure IOAPIC interrupts are
only routed to CPUs that don't have >255 APIC IDs. However, there is
an issue with kdump, because the kdump kernel can run on any CPU, and
hence IOAPIC interrupts can't work if the kdump kernel run on a CPU
with a >255 APIC ID.
The kdump issue can be fixed by the Extended Dest ID, which is introduced
recently by David Woodhouse (for IOAPIC, see the field virt_destid_8_14 in
struct IO_APIC_route_entry). Of course, the Extended Dest ID needs the
support of the underlying hypervisor. The latest Hyper-V has added the
support recently: with this commit, on such a Hyper-V host, Linux VM
does not use hyperv-iommu.c because hyperv_prepare_irq_remapping()
returns -ENODEV; instead, Linux kernel's generic support of Extended Dest
ID from David is used, meaning that Linux VM is able to support up to
32K CPUs, and IOAPIC interrupts can be routed to all the CPUs.
On an old Hyper-V host that doesn't support the Extended Dest ID, nothing
changes with this commit: Linux VM is still able to bring up the CPUs with
> 255 APIC IDs with the help of hyperv-iommu.c, but IOAPIC interrupts still
can not go to such CPUs, and the kdump kernel still can not work properly
on such CPUs.

On ARM64, Hyper-V now specifies the interrupt to be used by VMbus
in the ACPI DSDT. This information is not used on x86 because the
interrupt vector must be hardcoded. But update the generic
VMbus driver to do the parsing and pass the information to the
architecture specific code that sets up the Linux IRQ. Update
consumers of the interrupt to get it from an architecture specific
function.

In the architecture independent version of hyperv-tlfs.h, commit c55a844f46
removed the "X64" in the symbol names so they would make sense for both x86 and
ARM64. That commit added aliases with the "X64" in the x86 version of hyperv-tlfs.h
so that existing x86 code would continue to compile.
As a cleanup, update the x86 code to use the symbols without the "X64", then remove
the aliases. There's no functional change.
