Mike Rapoport d3c251ab95 arch, mm: remove stale mentions of DISCONIGMEM
There are several places that mention DISCONIGMEM in comments or have
stale code guarded by CONFIG_DISCONTIGMEM.

Remove the dead code and update the comments.
